• Ensure that all your current pets are up-to-date with all their vaccines, and include current vet’s contact information
• Make sure to include your landlord’s phone number (if applicable)
• Let us know who you are interested in, or what type of dog you would like
• If you’ve previously adopted from PUP, please let us know
• After your application is approved, make sure all family members, (and your current dogs) are present during the meet-and-greet with the potential new dog
